After receiving the insurance documents, read the following form carefully.
The documents are presented to the patient only in one copy, and if the copy is lost, it will not be issued again.
It is equivalent to make original copies of documents to submit to supplementary insurance.
The patient’s bill must be attached to the medical documents. Please bring it with yourself.

After 15 days from the date of discharge, the medical records unit will be contacted at 02175254286 and the patient will be given an appointment and it will be emphasized that they will come with the bill and the patient’s identification card to get the documents (either the patient himself or the 1st degree relatives).
